Jingle Bucks '12 Rules:
1. Based on IRS regulations, ticket purchases do not qualify as charitable donations. Payments are not tax-deductible.
2. Payments may be made by check, money order, MasterCard or Visa. Checks/money orders should be made payable to Partners For Community Living, 1651 Needmore Road, Dayton, OH 45414. Credit card payments may be made online at our secure website at www.partnersohio.com. Confirmation of ticket purchase will be mailed within 5 working days after receipt of payment. If a payment is dishonored, Partners will use reasonable efforts to notify the entrant by mail, phone or email. If unsuccessful, the entry will be invalid unless payment is confirmed prior to the drawing.
3. By submitting a completed registration form and accompanying payment, entrants accept the rules as stated.
4. Entry forms and payment must be received by Partners by 3:00 p.m. on Friday, December 7, 2012 or when 500 tickets are purchased, whichever comes first.
5. Entrants must be 18 years of age or older.
6. Partners employees and immediate families are not eligible to participate.
7. The name of the winner will be displayed on the Partners website, newsletters and other such promotion as deemed appropriate.
8. Entrants odds of winning will be determined by the number of tickets sold. Only 500 tickets will be sold. In the event 400 tickets are not sold by December 7, 2012 the winner will receive 50% of the money from the total amount of tickets sold. All entry forms will be recorded/accepted on a first come, first served basis. The winner is responsible for all applicable taxes.
9. The winner will be determined by random drawing on Friday, December 7, 2012 at approximately 3:00 p.m. at Choices In Community Living. The public is invited to attend. Winner need not be present to win.
10. Proceeds go to Partners For Community Living, in support of people with developmental disabilties served by Choices In Community Living and Resident Home Association.
11. More than one name may be listed on the entry form, however, the first name listed will be considered the "entrant." The Social Security Number must be that of the "entrant." Payment shall be made solely to the "entrant." It is the sole responsibility of the "entrant" in any multiple entry to allocate the prize among the participants in the multiple entry.
12. The decision of Partners For Community Living shall be final on all matters regarding Jingle Bucks '12
13. Jingle Bucks '12 is void where prohibited by law.
